We specialize in the care and management of geriatric populations and deliver exceptional care to private homes, senior living communities, and assisted living facilities.
We are currently seeking a Per Diem/Part-Time Nurse Practitioner or Physician Assistant to join our mobile care team. In this role, you will provide in-home health evaluations, build meaningful relationships with patients, and help bridge gaps in care.
Role Overview
The typical visit lasts about 45 minutes, allowing time to connect one-on-one, review medications and medical history, answer questions, and complete a physical evaluation.

What You’ll Do
- Provide compassionate, patient-centered in-home health evaluations
- Deliver comprehensive assessments, including medication review, medical history, and physical exam
- Engage with patients in their homes, assisted living, and senior community settings
- Build trusted relationships to support better health outcomes
- Travel locally within Los Angeles and Orange County
- Maintain accurate clinical documentation
- Communicate effectively with the care team as needed

- Active NP or PA license in California
- Valid driver’s license and reliable transportation
- Experience with geriatric or chronic care populations (preferred but not required)
- Strong communication and interpersonal skills
- Ability to work independently
This is a remote position.
Compensation: $45.00 - $60.00 per hour
